Horizon offers experienced diabetic nurses to teach on disease, assist with medication management, and to monitor progress.  We offer a special diabetic program for all of our  patients with this diagnosis.  Each patient is issued a diabetes pocket book designed by Horizon specialists to assist you in managing your diabetes. 

We believe diabetes is a managable disease state and our nurses are committed to assisting you to live a healthly life with your diabetes under control.
