Horizon Hospice offers an interdisciplinary team of specially-trained hospice professionals and volunteers working together to meet the physiological, psychological, social, and economic needs of patients, families, and significant others facing terminal illness and bereavement.

Hospice services are offered to any person who has been diagnosed with a terminal illness, who is no longer seeking active treatment for the illness, and who has a prognosis of six months or less. For individuals not yet eligible for Hospice, Horizon offers a Palliative Care Program (see Palliative Care).

Horizon's Hospice allows clients and their loved ones to be cared for in the comfort of their home, contracted nursing homes, assisted living facilities, and hospitals.

Services Offered:

• Medical Director to coordinate with primary physician
• Specially-prepared hospice nurses
• Social Workers to provide support and counseling
• Home Health Aides to provide personal care
• Music Therapy to calm and relax patients
• A Bereavement Coordinator to assist family and friends through the grieving process
• Grief counseling for one year following the death of the loved one
• Spiritual counseling by a chaplain
• Volunteers dedicated to helping patients and their families
