This fast-paced, newly created part-time position is an opportunity to combine your nursing experience, stellar interpersonal skills and ability to efficiently process paperwork.  We are looking for a bright, dynamic nurse who believes in the philosophy of hospice.  Build relationships and collaborate with hospital staff to make the admission process as smooth as possible. Help our new hospice patients and their families navigate the transition into hospice care.

The Care Coordinator for Hospice must be a self-starter able to perform with minimal supervision. Work independently but also collaborate with the hospice team on the admission process for new patients (data collection, eligibility and insurance verification, etc.).

This position requires outstanding customer service and interpersonal skills as well as a solid health care knowledge base. Must be computer proficient. RN degree and RN license required; prior hospice experience stronlgy preferred. Prior discharge planning or case management experience desirable.

This challenging position based out of our Brown Deer office assesses, arranges, and monitors home services for the elderly in Milwaukee County.  Work with a close-knit team of compassionate professionals committed to improving geriatric quality of life.  Full-time opportunity with NO weekends or On Call, plus excellent benefits! 

The minimum requirements to be a Case Worker are as follows:

1) Be a social worker certified in Wisconsin and

2) A four-year bachelor’s degree in a social services area (e.g. social work, rehabilitation, psychology, etc.) as well as at least one (1) year of paid formal (non-family setting) experience working with specific populations (e.g. DD, PD, Geriatric) in social service care management or related social service experience with persons in the CMO’s target population, or;

3) A four-year bachelor’s degree in any other area with a minimum of three (3) years of paid formal (non-family setting) experience with people in the CMO’s target population.

Seeking detail-oriented office professional with superior customer service skills. This important role coordinates the screening and intake of Home Care, Supportive Home Care and Hospice referrals. Assists the Intake Home Care Coordinators by inputting referral information, distributing referrals and verifying insurance coverage. The IS User Support Coordinator serves as an employee resource for training and troubleshooting with Microsoft Office applications and Windows Operating Systems.  Responds to user requests for assistance with hardware and software issues.  Responsible for hardware and software support for desktop and laptop computer users, user account maintenance, equipment inventory and asset management.  Assists with day-to-day computer issues such as password maintenance, account lockouts, printing issues, etc.  Performs routine maintenance of laptops and desktops and ensures availability of replacements.

Qualifications include a High School Diploma or Equivalent. Further Vocational/Technical education strongly preferred. Requires 1-2 years minimum of related professional experience.  Demonstrated knowledge of Windows Operating Systems, Microsoft applications, and network infrastructure and software are essential.

Our Hospice team is looking for a caring Medical Social Worker to provide initial psychosocial evaluations and ongoing psychosocial counseling to our patients and their families. This position is an opportunity to collaborate with an interdisciplinary team, work with the patients in the comfort of their own homes, and really make a difference.

 

Full-time position will require at least 2 days per week of on-call from 4:30pm - 8:00am, with the potential for more as desired.

 

Minimum qualifications include a Bachelor’s Degree in Social Work or related human services field from an Accredited School or University, plus current Licensure or Certification in Social Work for the State of Wisconsin. Must have a working knowledge of the principles and practices of social work, awareness of community resources, ability to relate to people of diverse cultural backgrounds, utilization of various treatment modalities, and the ability to work within an integrated team concept. Must have at least two (2) years experience in a clinical setting, preferably in either a health care setting or hospice. Current WI driver’s license, reliable transportation, and proof of current automobile insurance also required.
