Horizon Home Care & Hospice was established in 1990 and today serves as the second largest full service home care agency and hospice in Wisconsin.

Mission

Horizon Home Care & Hospice, Inc. will optimize health and quality of life for patients & families in the home & other settings. Our clinical experience, relationship centered care, and respect for patient rights set us apart as the region’s employer and provider of choice.

Our philosophy of compassion & responsiveness extends from our employees, to our hospital sponsors, and physicians, and to the patients and families we are privileged to serve.

Horizon is a private, not-for-profit organization sponsored by Froedtert Memorial Lutheran Hospital, Columbia St. Mary's and Community Memorial Hospital. We are licensed to provide home care and hospice services, are Medicare and Medicaid certified and accredited by The Joint Commission since June of 1999. The Joint Commission conducted an on site survey in June 2008. At that time, our agency was re-accredited. Our next accreditation survey is planned for 2011.

We welcome and encourage patients/customers to contact us with concerns related to patient care or safety. We serve patients in southeastern Wisconsin, averaging a census of 1,000 patients served by a staff of more than 550 employees.

Horizon's Home Care goal is to promote the ability of patients to care for themselves to sustain life and health or recover from disease or injury. Our Hospice program focuses on optimizing quality of life and providing support for patients and those involved in their care.
